FreeStyle® Libre 3 Plus added to BC PharmaCare formulary
Friday, Dec 12, 2025As of August 20, 2025, BC PharmaCare has added a flash glucose monitor (FGM), the FreeStyle® Libre 3 Plus device to its formulary with Special Authority (SA) approval.
Pacific Blue Cross will align the FreeStyle® Libre 3 prior authorization criteria with BC PharmaCare special authority criteria. The FreeStyle® Libre 3 Plus is the fourth glucose monitoring system listed by PharmaCare and joins Dexcom G6/G7 and FreeStyle® Libre 2.
Pharmacies will be able to submit FreeStyle® Libre 3 Plus claims electronically via the PharmaCare published PINS:

Similar to the FreeStyle® Libre 2, BC residents will be required to obtain BC PharmaCare SA approval in order to be eligible for FreeStyle® Libre 3 Plus coverage. Once approval is in place, claims will automatically be approved based on plan rules. Claims no longer need to be submitted manually by patients.
Patients residing outside of BC may fill out the PBC Dexcom G6/G7/FreeStyle® Libre 2/3 Plus request form.
