Live well with chronic conditions using free Self-Management BC program

Are you taking medications for diabetes or high blood pressure, or have a chronic condition? Explore free programs offered by Self-Management BC to help manage these conditions.

Long-lasting health diseases that require ongoing medical care and lifestyle changes can be defined as chronic conditions.

Having the skillset and knowledge to live well is called self-management. The goal of self-management is to empower you with confidence to thrive while managing chronic conditions like diabetes, achieved through learning the roles of physical activity, nutrition and medication.

Program options

Self-Management BC’s programs are offered in person, virtually, by mail and by telephone, and are available for English, Chinese, Punjabi and Indigenous communities.  Programs are free for participants and are government funded.

Self-paced learners will benefit from the by mail program option. Participants will receive a one-time mailing of program materials that includes resource materials, including a book and a self-assessment booklet.

Participants who’d like to build community and gain support in a group setting will enjoy the in person or virtual option. In-person programs are held in communities throughout British Columbia (BC), with a maximum of 12 participants. Virtual programs are conducted via webcam with a maximum of 8 participants. Both in person and virtual programs run for 6 weeks, with weekly 2.5-hour sessions.

The online program offers a hybrid option. Participants can log in on their own time, but also be part of a supportive group that helps each other stay on track.
